Job DescriptionManager, Community IntegrationPermanent Full- TimeReporting to the Program Director, Community Integration, the Manager, Community Integration is responsible for all activities related to the administrative and patient care functions within the designated portfolio. The Manager will work with Clinical and Program leaders and Community Partners to ensure activities and programs are consistent with Mackenzie Health (MH) and the WYR-OHT goals/objectives and that efficient/effective administrative and clinical practices are provided.The portfolio will include oversight of the Mackenzie Helps @Home Program, Transitional Care Unit, Mackenzie Health Long Term Care Home and the Nursing Led Outreach team (NLOT). This portfolio also includes the ever-growing operations of the WYR-OHT as it progresses towards maturity and expands to include new functions of the WYR-OHT such as assuming Home and Community Care contract oversight and Primary Care Network expansion.The Manager is responsible for the operation, organizational effectiveness and oversight, and financial management of contracted out integrated community service programs. This role will ensure the development and maintenance of a collaborative approach to contract management with health service providers including the oversight of key performance deliverables and transfer payment agreements with Ontario Health. This position works in partnership with Mackenzie Health Patient Care Managers, Support Services Managers and Community Health Service Provider Managers to ensure that high quality, effective and efficient services, and care is provided. The Manager Community Integration is accountable for planning, implementing, and integrating plans that are in alignment with both Mackenzie Health and the WYR-OHT ’s vision and strategic directions.PRIMARY RESPONSIBILITIES
- Acting as lead hospital representative for the planning, co-ordination and day-to-day contract administration of integrated community service programs.
- Responsible for working collaboratively with contracted service providers and community partners to ensure the effective management and operation of community integration services, including the joint development and implementation of short and long-term strategic
- Promoting team spirit, cohesiveness, motivation, and commitment to patient care excellence through engagement, empowerment, and transformational leadership.
- Reviewing and revising policies and procedures, including the development of any required contract addendums, request for proposals and quarterly submissions to Ontario Health, to ensure excellent service delivery that aligns with Best Practice and patient outcomes.
- Working collaboratively with PCMs, partner organizations and key interdisciplinary members to address operational issues that impact the delivery of service and the efficient use of resources.
- Ensuring excellence in patient care and safe service delivery through continuous quality improvement processes that include planning, quality monitoring and risk avoidance.
- Providing leadership in the development of new initiatives by creating an environment that encourages creativity, innovation and critical thinking across various interdisciplinary teams and community
- Contributing to the organization’s vision, values and strategic direction that align with organizational

QUALIFICATIONS
- Regulated Health Care Professional registered to practice in Ontario.
- Minimum of 2 years relevant experience as a clinician in a hospital or community setting with a strong clinical background in primary care, chronic disease management and population health.
- Progressive leadership experience in quality, resource and utilization management and planning, capacity building and collaborative partnership development.
- Previous experience at the Manager level preferred.
- Master’s level preparation in Health or Business Administration or equivalent preferred.

- Valid driver’s license and reliable vehicle to support regular travel between all three sites: Reactivation Care Centre, Mackenzie Richmond Hill Hospital and Cortellucci Vaughan Hospital and WYR-OHT Catchment area.
- Proven attendance record.
WORK ENVIRONMENT
- This position is an on-site position.
- Potential need to travel throughout Western York Region.
- You may be required to work at any site of Mackenzie Health.
