Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityReporting to the Health Services Manager, Palliative Navigator, through an intake process, identifies clients/family needs, develops recommendations, and evaluate the most appropriate services and programs that will assist in meeting the frail elderly/complex case individual/family’s goal in living in their own community for as long as possible. Works collaboratively with the client’s family physicians, multi-disciplinary health care team and community supports/agencies. Supports access to the services and programs that enables successful pain for the client/family. The ultimate goal is to enable the client to live with support to successfully meet personal goals of care.About YouWe would love to hear from you if you have the following:

  • Degree in a healthcare discipline (e.g., nursing, occupational therapy, physiotherapy, social work)
  • Registered or eligible for registration with correspondence college or governing body
  • Three (3) years’ experience working with Palliative care and community organizations and services. Experience working in health care system
  • In-depth knowledge and experience with community practice and frail elderly population
  • In-depth knowledge of Nova Scotia Health Authority services/programs and specifically in the community and health care facilities.
  • Excellent verbal and written communication skills
  • Ability to develop and maintain community networks.
  • Ability to use data in the evaluation and improvement of the Geriatric Navigator.
  • Able to be innovative and creative to develop strategies that successfully meet the needs of diverse client, family, and provider populations.
  • Competency in computer-based programs (Microsoft Office Programs)
