Center for Life Management (CLM) provides the area’s most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness, co-occurring developmental disabilities and substance use disorders. As the Associate Director of the Community Support Program this position will be responsible for Case Management, Housing and Referral Education Assistance & Prevention (REAP) programs. The Associate Director will oversee and coordinate the delivery of comprehensive case management services to individuals with mental health disorders. The role will involve strategic planning, team leadership, and ensuring high-quality care coordination to optimize patient outcomes and satisfaction. This is a full time, 40 hour per week position.

Major Functional Areas and Description of Duties

  • Supports the Director of the Community Support Program with the following:

  • Development of treatment modalities consistent with federal and state regulatory standards and statues to ensure evidence-based, culturally competent, trauma-informed care is outcomes-driven and of highest possible quality;

  • Works with Bureau of Mental Health Services (BMHS) in service system planning for adult programming;

  • Management of resources, priorities and caseloads of multi-disciplinary teams, treatment planning, and case conferences for all clients, as needed;

  • Provides new staff with orientation and ongoing administrative training opportunities within the department;

  • Provide formal, regularly scheduled supervision and supervisory support as needed to assigned supervisee’s;

  • Provide ongoing feedback on performance and completes formal performance reviews of assigned staff;

  • Utilizes outcome measures to identify areas of continuous quality improvement;

  • Assists in data-driven supervision and skill building of program staff;

  • Ensures that all documentation is completed accurately, on time and in compliance with CLM, federal, state and other pertinent standards;

  • Monitors program implementation and authenticity by adhering to and upholding the Evidenced Based Practices;

  • Oversees and sustains service delivery, contracts and associated grants related to service delivery practices;

  • Ensures service delivery is efficient and effective by developing methods to meet desired client outcomes, Key Performance Indicators (KPI), and budget variances;

  • Supports the expansion of service delivery within department by implementing services and programs in line with evolving agency needs.

  • Ensure self and team members:

  • Execute all required services and documentation in accordance with agency policies, federal standards, and state statues;

  • Work collaboratively with other treatment team members, natural supports, and external providers to ensure wrap-around services for continuity of client care;

  • Serve on appropriate committees, both internal and external, as determined by supervisor;

  • Function professionally and in a manner that protects the integrity, confidentiality, and rights of all clients;

  • Promote a positive work environment and culture that fosters cooperation, teamwork, professional growth and development to benefit CLM staff, clients, families, and community partners;

  • Maintain professional memberships, licenses, accreditation, certifications, etc.;

  • Adhere to Center’s Code of Ethics;

  • Represent CLM in a professional and appropriate manner in all settings;

  • Perform all other duties as assigned by supervisor.

    Qualifications and Skills

  • Master’s degree in Social Work, Psychology, Counseling, or related field;

  • Minimum of four years supervisory experience;

  • Proven experience in case management within a mental health setting, with progressive leadership responsibilities;

  • Strong leadership and management skills, with the ability to motivate and inspire a diverse team;

  • Previous experience providing crisis intervention, working with co-occurring disorders, and working within a multi-disciplinary team;

  • Ability to relate well to staff, model conflict resolution skills and nurture a supportive work environment among all staff;

  • Demonstrates a high level of emotional intelligence to manage complex situations to include exercising sound clinical judgment in routine, urgent, and emergent clinical and staffing situations;

  • Ability to document effectively and efficiently in course of clinical work;

  • Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision;

  • Effective communication skills, organizational skills, problem-solving skills and ability to multi-task;

  • Knowledge of mental health treatment modalities, recovery principles, and community resources;

  • Proficient computer skills and knowledge of all Microsoft office applications (Word, Excel, Publisher, Power Point, Outlook, TEAMS, etc.);

    Physical Demands and Working Conditions

The work will be office based. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Must be able to flex schedule to accommodate needs of clients and staff. For community-based work or meetings, reliable transportation, a clean driving record, and proof of auto insurance coverage are required.
