With an increased demand for our services, we have an exciting opportunity for an experienced care manager to join us in our mission to enhance the lives of aging adults and their families.The position of Client Care Manager preforms a variety of duties including consulting with potential clients and family members, developing individualized care plans, introducing Care Professionals to clients, conducting regular quality assurance visits, and documenting and updating care plans to ensure all aspects of client needs are met. The Client Care Manager actively promotes our services and develops strong relationships with health care providers and others who support and advocate for seniors in our community.Primary Responsibilities

  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan
  • Conduct Service Inquiries and Care Consultations as needed following the consultative sales process
  • Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service
  • Work with other team members to coordinate various aspects of a client’s care
  • Introduce Care Professionals to clients and ensure they have a full understanding of a client’s care needs and preferences
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program
  • Conduct regular quality assurance meetings with clients in person or via phone
  • Demonstrate open and effective communication with clients, family members, referral providers, and internal support team
  • Build strong relationships with existing and potential referral partners to promote and grow our business
  • Maintain accurate records of all sales and prospecting activities
  • Accountability for achieving clearly defined key performance indicators
  • Participate in various PR strategies

Education/Experience Requirements

  • College degree (RN or LPN preferred but not necessary)
  • One year of experience in home care, health care, or senior-related industry is required, an equivalent combination of education and work experience may be considered
  • Must possess a valid BC Driver’s license and own a vehicle with Business Insurance coverage
  • Participate in Back-up On Call phone rotation to assist in urgent matters

Compensation: $55,000-$65,000 per year
