This role will be responsible for assessment and caseload management of those referred into Mental health services working as part of a stepped care model. You will completing comprehensive assessments, confidently communicating treatment options and initiating supportive care plans, and providing evidence-based interventions alongside the wider multi-disciplinary team.You will work with internal and external partners, including partner healthcare providers, prison colleagues, social care providers, probation services. As such, you will need excellent communication skills and a teamwork ethos.Main duties of the jobYou will be a Registered Mental Health or Learning Disability Nurse with at least 18 months experience within a B5 or B6 role. You’ll have a flexible approach to working within a team that encourages new ideas and opinions on how to improve the service.You will have experience providing compassionate mental health care and have an understanding of the criminal justice system. You will make on going assessment of need reflecting individual circumstances and provide relevant interventions and input to discharge planning. You will be required to have strong coordination and planning skills, with the ability to make decisions based on comprehensive health and risk assessments.You should be innovative, flexible and able to engage with patients with complex needs. You will be committed to a person-centred approach and have the ability to prioritise.Please note this post has £3,000 joining bonus alongside development opportunities for successful applicants.
