As a recognition of our commitment to quality and patient safety, we have received the highest distinction of Exemplary Standing from Accreditation Canada.Job SummaryThe Registered Nurse (RN) role models clinical excellence and the core competencies of the practice standards of the College of Nurses of Ontario (CNO), as well as the policies and procedures of Southlake Regional Health Centre (Southlake). The RN works collaboratively as a member of the health care team. The RN provides direct patient care to patients based on the three-factor framework as outlined in the College of Nurses of Ontario standard, RN and RPN Practice: The Client, Nurse and the Environment. These three factors have an impact on decision making related to care-provider assignment as well as the need for consultation and collaboration among care providers. All nurses make a commitment to lifelong learning.The successful candidate will be motivated, self-directed and a key member of a dynamic interdisciplinary team with defined roles focused on quality patient care and patient flow. The successful candidate will be responsible to provide excellent care to a diverse acute medical patient population in a fast-paced setting.The Clinical Nurse Consultant (CNC) is a unique and advanced position that involves working within community-based inter-professional Palliative Care teams to ensure the delivery of effective, high quality care to patients living with a life-threatening illness and their families in their home setting.The CNC will provide expert mentorship and support to primary clinicians, nurses, and other members of the health care team to enhance palliative care knowledge and capacity within the community.As Lead Educators for Hospice Palliative Care in the community, the CNC will provide accredited education courses to health care professionals across the Central LHIN.As a member of HPC Teams for Central LHIN, the CNC will participate in the provision of a 24/7 on call consultation service with an appropriate level of physician back up support.Primary Responsibilities

  • Provides patient and family focused care within the RN scope of practice
  • Functions in accordance with established hospital policies and procedures, including emergency codes
  • Ensures that practice is consistent with entry to practice competencies, standards of practice, guidelines and legislation
  • Takes a leadership role in the coordination of patient care in assigned area
  • Provides face to face or virtual visits in patient homes, long term care homes, retirement homes and other group settings in the community
  • Communicates effectively with patients, families and all members of community and hospital healthcare teams
  • Practices collaboratively with members of the healthcare team to provide elements of care for patients/clients with complex and life-limiting health issues
  • Provides consultation and/or clinical support to health care professionals and peers based on best practice as identified in the Model to Guide Hospice Palliative Care
  • Provides education to health care professionals both at the patient bedside, larger classroom settings and virtually
  • Performs a detailed assessment of each patient’s health status on an ongoing basis and documents within CNO guidelines
  • Assists in the development and implementation of the Plan of Care and evaluates its ongoing effectiveness
  • Assists in the orientation/preceptorship of new staff members and students to the clinical team
  • Acts as a palliative care role model and champion to identify and integrate safe, best practices into daily activities to foster the delivery of safe and exemplary care
  • Facilitates monthly interprofessional community rounds in an assigned geographical region
  • Effectively utilizes human and material resources
  • Participates in the development, implementation and evaluation of clinical pathways and educational modules
  • Other duties as assigned by Manager/delegate

Qualifications

  • Registered and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
  • Bachelor’s Degree in Nursing required. Masters in a healthcare related field preferred
  • Current Canadian Nurses Association (CNA) Certification in either Hospital Palliative Care or Oncology or the applicant meets, at the time of the job application, the CNA Certification eligibility requirements to write the CNA Certification exam (the applicant will normally be required to proceed to obtain certification within 9 months of employment unless an extension for obtaining the certification is approved by management).   • Member of RNAO with current malpractice and legal coverage
  • Must possess own vehicle, a current Ontario Driver’s License and a clean driving record
  • Ability to maintain a private and secure home office
  • Minimum of three (3) years of direct palliative care experience in the last 5 years
  • Proven experience with direct palliative and End of Life care with patients of all ages
  • Experience in delivering hospice palliative care education to health care professionals, both individually and in group settings
  • Ability to work both independently and cooperatively with other members of the community-based interprofessional and primary care teams
  • Able to work in a self- directed environment, meeting program targets and Home and Community Care Support Services deliverables
  • Excellent clinical decision making skills with an expertise in palliative pain and symptom management
  • Demonstrated advanced knowledge base of pharmacological and non- pharmacological interventions for pain and symptom management
  • Excellent physical assessment skills
  • Demonstrated knowledge of Advance Care Planning and Goals of Care in Ontario
  • Excellent computer literacy – Microsoft Programs, e.g. Outlook, Word, PowerPoint, Access
  • Comprehensive knowledge of community health services within Central Home and Community Care Support Services
  • Innovative and willing to learn

  • Demonstrates excellent organizational, time-management and multitasking skills
  • Demonstrates initiative and the ability to function both independently and as an interprofessional team member in a fast paced, demanding environment

  • Good working knowledge of Electronic Health Care Documentation
