End of Life Care within Walsall is rated as OUTSTANDING by CQC within community and the Hospice. The post holder will work within a multidisciplinary team who are focused on delivering the best holistic care possible to patients with a life limiting illness.Goscote Hospice located at Goscote Lane, Palliative Care Centre is integrated with the Palliative & End of Life Care services provided by the Trust, which include community nursing, day hospice and hospital and community palliative care nursing , Consultants, Allied Health Professionals.   • Caring for patients with a life limiting illness likely to be in the last 12 months of life
  • Support symptom control management for patients attending with physical, psychological , emotional symptoms
  • Care for patients in the last days/weeks of life
  • Care for patients admitted for symptom management
  • Be part of wider Specialist Palliative Care MDT
  • Deliver fundamentals of care based on patient wishes
  • Support communication with carers
  • Link with teams across acute , community & primary care
  • Clinically lead the team on a day to day capacity and in the absence of the Band 7 lead nurse
  • Complete quality audits
