Detailed job description and main responsibilitiesDirect care of patients attending the Physical Healthcare Centre or in the ward environment to carrying out clinical interventionsAct as a contact for internal and external stakeholdersProviding and receiving routine information which requires tact or persuasive skills or where there are barriers to understandingAbility to maintain a professional therapeutic relationship with patients, using tact, persuasive and reassurance skills and where barriers to understanding exist, adopt alternative methods to help overcome these barriersMonitor and manage the hospital high dose antipsychotic drug monitoring schedule, ensure all patients are offered blood tests and ECG within a pre agreed time scale, escalating any concerns to the nurse in charge of the shift or GP’s.Please note that this post does not meet the pay level required for a Skilled worker visa.Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa.   • NVQ L2/3 H&SC or Diploma L2/3 in H&SC or apprenticeship L2/3 in H&SC. Venepuncture Obtaining a spirometry Obtaining an ECG

Desirable criteria

  • VRQ L3 in mental health

ExperienceEssential criteria

  • EPrevious experience within the primary care sector. Experience using system one patient management software Experience management a project. Experience in supporting patients manage their long term conditions using quality outcome framework.

Desirable criteria

  • Experience of working in Forensic Mental Health setting

KnowledgeEssential criteria

  • Awareness of professional boundaries in relation to client care. Understanding of issues surrounding confidentiality. Awareness and understanding of equality and diversity issues. Well-motivated with an interest in mental health issues.

SkillsEssential criteria

  • Record keeping and report writing skills.
  • Good written/oral communication skills
  • IT skills including use of patient information systems, with ability to learn to effectively utilise service specific computer packages.
  • Ability to work without direct supervision, liaising appropriately with other practitioners in the team.
  • Using initiative whilst liaising with other team members.
  • Good level of self-awareness and interest in self-development.
  • Able to motivate others.
  • Respectful of service users’ individual needs and requirements within their own environment.
