_Position Summary_:
The Case Manager, MSW integrates the duties as case manager by performing discharge planning evaluation of assigned patients. The Case Manager, MSW/BSW is responsible for care coordination and development of the discharge plan through collaboration with the healthcare team and internal/external resources. MSW/BSW functions as a primary contact for coordination of internal and external resource services to facilitate care delivery in the discharge plan. As a member of the healthcare team they communicate effectively with the interdisciplinary team to ensure safe, quality care is provided to all patients.
Essential Functions
1. Effectively performs as a case manager as evidenced by active participation in the interdisciplinary team plan of care.
2. Demonstrates the ability to increase awareness of existing services and community resources to the patient, family, and caregivers.
3. Demonstrates the ability to effectively communicate recommendations of the interdisciplinary team to the patient, family, caregivers and other support systems.
4. Effectively interviews patient, family members and other support systems to access clinical status of patients scheduled for discharge. Formulates a discharge plan in a timely manner that matches patients with organizations, settings and services best able to meet the needs of the patient. Provides ongoing assessments during hospitalization to ensure appropriate referrals based on status at the time of discharge.
5. Demonstrates the ability to integrate age specific factors into the plan of care and involves patient and family into the care plan process.
6. Case Manager, MSW/BSW independently conducts and documents discharge screening/evaluations within 24 hours of admission.
7. Facilitates and coordinates patient/family conferences as applicable.
8. Effectively counsels patients and their families regarding psychosocial needs or problems.
