Utilizing a personal outcome-based approach, this position supports people in making informed choices that lead to successful inclusion and participation in community based recreational/leisure activities and work/volunteer placements.Compensation Package:

  • Work up to 37.5 hours per week;
  • Minimum starting rate of pay is $28.73 per hour;
  • 11 statutory holidays are observed;

Following the successful completion of a 3-month probationary period:

Minimum Requirements for this position include:

  • Degree or Diploma in developmental services or a closely related field; or a combination of education and experience which provides the required knowledge
  • One-year recent full-time work experience working in a community setting supporting the integration of people supported through outcome-based approach;
  • Current valid certificates in First Aid, C.P.R. and N.V.C.I.;
  • Demonstrated knowledge of community resources;
  • Excellent written and verbal communication skills;
  • Good computer skills with experience in Microsoft office applications and database administration;
  • Good interpersonal skills;
  • Understanding of The Council on Quality and Leadership and Personal Outcome Measures.

Special factors of this position:

  • Needs of this program require flexibility as support needs are based on people’s individual needs and community partner requirements;
  • Will be required to work flexible hours which will include working evenings, weekends and public holidays;
  • Current valid driver’s license and own reliable transportation to be used in the course of employment.

Duties and Responsibilities (does not include all duties):

  • Facilitating and supporting participation in meaningful community-based activities which support people to develop skills utilizing community infrastructure such as transportation services, to promote and develop leisure, recreational, employment and volunteer opportunities within the community;
  • Participating in goal planning and attending individual planning meetings as requested and supporting people to attain their desired goals;
  • As a member of a support team, identifying and accessing additional resources needed to support people in the community;
  • Providing personal support and assistance in the community which are of interest and appropriate to the needs of the person supported;
  • Facilitating positive involvement and interaction in the community, acting in a professional manner at all times and as a role model;
  • Assisting people to maintain positive relationships in volunteer placements; providing job coaching as required; Supporting people to successfully transition from school;
  • Knowledgeable of individual program procedures for administering medication in order to dispense medications as needed;
  • Maintaining a system of reporting and communication of all relevant information to those involved in the supported person’s outcome-based support plan;
  • Ensuring proper safety measures and awareness training is provided to people supported to ensure they recognize and respond appropriately to any safety issues;
  • Providing personal hygiene supports as necessary;
  • Being respectful and maintaining confidentiality of people’s personal information at all times;
  • Ensuring proper use of and maintenance of Agency equipment, buildings, property and other assets;
  • Demonstrating understanding of and compliance with legislative knowledge of the Employment Standards Act, Human Rights Act, Collective Agreement (if applicable) and other Acts and Legislations as it applies to the role;
  • Performing other duties or special assignments as assigned.
