Consultant in Public Health & Head of the PHM HubThis is an exciting opportunity for a healthcare public health consultant with a strong interest in policy to lead population health-based transformation at one of the largest clinical academic teaching hospitals in the country. The role involves driving change across GSTT in collaboration with its partners to move towards more equitable, preventative, and population health-based models of care, fit for the mid-21st century.As the Consultant in Public Health and Head of the Population Health Management Hub, you will lead the work of the Population Health Management Hub, providing clinical leadership for the Trust’s vision for population health and health equity. Supported by the Chief Medical Officer, Deputy Chief Medical Officer (Healthcare Public Health), and Senior Clinicians and Managers working in Public/Population Health in the Trust, you will work in partnership with Public Health Teams in Local Boroughs. Your responsibilities will include leading the design, development, and delivery of the Trust’s Population Health & Health Equity Programme, in line with the strategic ambitions of GSTT and its Partners, such as KHP, and aligned with the strategic interests of South East London Integrated Care Board.The role is unique and highly technical, combining population health practice and policy with the delivery of strategic change. You will need experience and knowledge from clinical practice to drive forward changes to service delivery models that positively impact population health and patient outcomes within the context of a healthcare provider. You will need to be innovative, collaborative, and driven to make a difference to the health outcomes of the populations served by GSTT, and possess strong leadership skills to influence others to deliver towards this goal.This is a 24-month appointment, funded by the Guy’s and St Thomas’ Charity, with the intention of securing ongoing and sustainable funding for the Population Health Management Hub.
