MTM Clinical Pharmacist
Job Description
The Medication Therapy Management Clinical Pharmacist is responsible for activities related to Medication Therapy Management (MTM), Adherence Outreach, Opioid Case Management, and/or other clinical programs.
Responsibilities
Perform comprehensive medication therapy review sessions with MTM eligible members; review member’s medication profile to identify, prevent, and resolve medication-related problems to help members maximize benefits and minimize cost; document the member encounter in the MTM platform
Provide medication education to members including, but not limited to, medication adherence, appropriate use of medications, and minimization of side effects to improve health outcomes; document encounter in appropriate platform
Review and discuss identified potential drug to drug and drug to disease interactions with members and health care professionals
Contact member’s provider to seek resolution on medication related problems if necessary
Contribute to projects as needed to support business needs
Collaborate with team members in MTM/CCM support roles by providing assistance and guidance as needed
Other duties as assigned
Minimum Qualifications
Doctor of Pharmacy (PharmD) or Bachelor of Science Pharmacy Degree
Current Pharmacist license in good standing with a State Board of Pharmacy
1 year of related work experience
Must be eligible to work in the United States without need for work visa or residency sponsorship
Additional Qualifications
Microsoft Office Suite proficiency
Analytical and problem-solving skills with attention to detail and quality
Ability to manage time while working on multiple projects simultaneously
Ability to develop process efficiencies
Ability to work well in team environment
Preferred Qualifications
MTM experience or completion of a pharmaceutical care training program
Pharmacy claims experience
Pharmacy Case/Care Management experience
Experience working with Medicare members
Minimum Physical Job Requirements
Ability to work a flexible schedule including weekends, holidays, overtime, on-call, and shifts outside of Prime’s core business hours of 9:00 a.m. to 3:00 p.m.
Constantly required to sit, use hands to handle or feel, talk and hear
Frequently required to reach with hands and arms
Occasionally required to stand, walk and stoop, kneel, and crouch
Occasionally required to lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus
Reporting Structure
Potential pay for this position ranges from $109,000.00 – $174,000.00 based on experience and skills. Pay range may vary by 8% depending on applicant location.
