The successful candidate must self-identify as African Nova Scotian/Black people of African Decent in order to be considered for this role.Reporting to the Primary Health Care Manager (PHC), the Primary Health Care Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management, and Department of Family Practice (DFP) portfolios specifically related to the health and wellness of Men of African Descent. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in the Nova Scotia Brotherhood portfolio.The scope of practice for the PHC Coordinator will include collaborative work with African Nova Scotian leaders and community. Liaise with health care providers to support change management strategies that are inherent in the services plan for primary health care. The PHC Coordinator has responsibility for supporting the overall coordination of the planning, development and implementation of the activities assigned under the direction of the leadership team. The PHC Coordinator is responsible for supporting the ongoing development and implementation of the Nova Scotia Brotherhood quality program. The PHC Coordinator activities are guided by PHC priority areas, multiple stakeholders and by community needs therefore, requires a professional approach. This work requires recognition and application of best practices and application of evident informed decision making. The PHC Coordinator serves as a resource to other team members in Primary Health Care and the Department of Family Practice as well as within Nova Scotia Health as required.Travel within the catchment area maybe required.About YouWe would love to hear from you if you have the following:
