Part Time Social Worker – Hospice Saratoga
The Community Hospice has a part time opportunity for a social worker to join our hospice care team caring for patients in our Saratoga/ Washington county nursing homes and inpatient settings for Saratoga Hospital and Glens Falls Hospital as part of our Interdisciplinary Team caring for patients and families receiving hospice care. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Hours may vary and flexibility is welcomed!

Responsibilities:
Diagnose and treat psychological, behavioral and emotional disorders as they relate to grief
Provide specialized skills and mental health therapy for children
Provide support groups in school setting and on hospice sites
Ensure patients receive appropriate resources in most appropriate setting
What you will need:
NYS SW Licensure
Valid NYS Drivers license
Minimum 2-3 years experience ideally caring for children and families
The Community Hospice provides a thorough orientation and a supportive work environment

Pay Range: $28.80 – $40.00
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
