The successful candidate must self-identify as a member of 2SLGBTQIA+ communities.prideHealth is a Primary Health Care initiative that works to improve access to health services which are safe, respectful, coordinated, comprehensive, and culturally safe for members of 2SLGBTQIA+ communities. prideHealth works in collaboration with other services and initiatives within Primary Health Care and portfolio partners across Nova Scotia Health Authority to offer programming and support.Reporting to the Primary Health Care Manager (PHC), the Primary Health Care Community Liasion will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management, and Department of Family Practice (DFP) portfolios specifically related to the health and wellness of the 2SLGBTQIA+ communities. The Community Liaison will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in the prideHealth portfolio.The scope of practice for the Community Liaison will include collaborative work with prideHealth leaders and community. Liaise with health care providers to support change management strategies that are inherent in the services plan for primary health care. The PHC Community Liaison has responsibility for supporting the overall coordination of the planning, development and implementation of the activities assigned under the direction of the leadership team. The Liaison is responsible for supporting the ongoing development and implementation of the prideHealth quality program. The Community Liaison activities are guided by PHC priority areas, multiple stakeholders and by community needs and requires a professional approach. This work requires recognition and application of best practices and application of evidence informed decision making and serves as a resource to other team members in Primary Health Care as required.Travel within the catchment area is required.About YouWe would love to hear from you if you have the following:
