Your Opportunity:As a member of the Community Care Registered Nurse Case Management Team you will be responsible for the assessment, design and implementation of continuing care services for a caseload of clients residing in the community. The Register Nurse will also be responsible, as a part of the team, for the provision of Registered Nurse consultation, assessment, care and health teaching to clients in various living settings as required related to your caseload and your team’s needs. Responsibilities: Comprehensive assessment, case management and intervention techniques. Awareness of current trends, issues, and practices in community care. Awareness of Community Resources. Current best practice in case management processes. Understanding of Quality Improvement theory and methodologies. Conflict resolution and crisis intervention. Leadership. Time management and organization.Description:As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour – $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management – Chapters 1 – 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0) Additional Required Qualifications:Home Care Case Management experience within the last 2 years. Knowledge of and skills in applying Home Care Nursing Practice Standards. Knowledge of Continuing Care Health Service Standards. Meditech care manager training. Family assessment skills. Effective oral and written communication. Working independently and as member of a team. Working with people in a health care environment with professionalism and respect. Positive approach and good listening skills in dealing with challenging situations with empathy and sincerity. Understanding of principles of Palliative Care Knowledge of Alberta Aids to Daily Living (AADL) Program. Ability to perform all assigned tasks and direct clinical care including but not limited to: Constant adjustment of position (sit/stand) throughout the shift. Physical exertion including bending/walking/sitting or standing for long periods of time. Keyboarding/moussing documentations for prolonged periods of time. Ability to perform all duties in accordance with related legislation.
