Job Code: 13052Status: Part TimeLocation: Providence Transitional Care CentreUnion: ONA PCHWage: $39.07 – $56Posting Period: Posted until FilledAs a member of an inter-professional team, the role of the Registered Nurse (RN) at Providence Care is to deliver safe, effective, patient centered care to assigned patients in a non-acute setting as outlined by hospital policies and procedures and the Standards of Nursing Practice of the College of Nurses of Ontario. Working at full scope of practice, the overall function of the RN is to assess, plan, implement and evaluate the clinical conditions and transition of the patient and to enhance the quality of life by meeting the physical, emotional, social and spiritual needs of each person.Skills and Qualifications Required:

• Active, unrestricted license to practice with the College of Nurses of Ontario
• Current CPR certificate
• Nursing Diploma from an approved university or college
• BNSc preferred

Position Responsibilities:

• Acts as a role model, preceptor and mentor for the profession and for other Registered Nurses, Registered Practical Nurses and nursing students, and is committed to the values of the nursing profession and to the mission, vision and values of Providence Care
• Works collaboratively with fellow co-workers and the leadership team toward the achievement of the Providence Care’s overall goals and objectives
• Evaluates the health status of patients with complexity of care needs and determines the initial plan of care in collaboration with the patient, family members and members of the inter-professional team
• Works with the inter-professional team in establishing priorities for care including preventive, therapeutic and rehabilitative aspects and participates in discharge planning
• Provides direct care to patients within his/her scope of practice in accordance with the College of Nurses standards, Regulated Health Professionals Act, the policies and procedures of Providence Care and all other relevant legislation
• Provides patients and their family with access to valid information and resources related to the patient’s care and goals and provides health teaching as necessary
• Develops a relationship of mentoring and teaching for other Registered Nurses, Registered Practical Nurses, nursing students and provides education and support in their learning
• Accountable and responsible for the identification of knowledge and skills necessary to achieve competency within the given scope of practice and participates in the ongoing evaluation of programs, processes and desired patient-centred outcomes
• Fulfils the requirements for practice standards mandated by the College of Nurses of Ontario regulatory body
