Concentrating on self-management, reducing health risks, healthy eating, physical activity, mental wellness and parenting, HealthyNS closely aligns its programs with the integrated wellness and chronic disease prevention and management strategy, Charting the Course. This strategy aims to develop a culture of self-management where Nova Scotians are the driver of their own health and wellness. It also aims to shift the culture to wellness, risk factor management and chronic disease prevention for all Nova Scotians.Reporting to the Provincial Chronic Disease Management and Wellness Health Services Lead, Primary Health Care, the Wellness Navigator will implement behavior change methods, principle of adult learning, and client centered approaches to provide self-management education and facilitate group wellness programing. The Wellness Navigator is team focused and collaborates with provincial team members, primary care providers, other health care providers and community groups through a community development, population health, and collaborative approach. The Wellness Navigator will demonstrate excellent leadership skills and initiative to solve problems, share responsibilities, and contribute to a positive team environment. They will work collaboratively with team members and community partners to develop, implement, and evaluate mental wellness and navigation programing that aligns with the strategic direction of HealthyNS. This will be a hybrid position, providing both virtual and in person programming. The incumbent may be required to travel to work in various community locations throughout the province.About YouWe would love to hear from you if you have the following:

  • Bachelor’s degree required from an accredited institution in one of the following health care disciplines: Occupational Therapy, Social Work, Registered Nurse, Recreation Therapy
  • Registration with respective professional regulatory association required
  • Minimum of 3 years’ experience including: clinical practice related to discipline; working in Primary Health Care setting/community-based team/community setting and minimum one-year recent mental health
  • Facilitation/education skills utilizing adult education, self-management supports, and behavior change principles (both individual and group settings)
  • Experience working with people living with a chronic condition(s)
  • Experience working with community groups/partners building trusting relationships through working together on projects/ initiatives /partnerships that benefit the public (professional and /or volunteer); individual and group education/ facilitation utilizing adult education, self-management supports. and motivational enhancement principles.
  • Contributes to a positive team environment within a virtual setting
  • Strong program development skills
  • Understanding of population health, health promotion strategies, and behavior change principles
  • In-depth knowledge of health authority and community-based services
  • Demonstrates person-centered/family-focused, diversity relevant approaches to care
  • Well developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability and willingness to change
  • Demonstrates innovation in program planning, implementation and evaluation, and quality improvement
  • Computer skills (MS Office, email, zoom for healthcare, and internet)
  • Annual CPR-AED certification and annual WHMIS training and certification
  • Valid driver’s license
  • Competencies in other languages preferred, French an asset
