Applications are invited for the post of Specialist Registrar (ST3+ Level) in Palliative Medicine. This is a fixed term post (September 2024- September 2025).West Suffolk Hospital is a busy and popular hospital, serving a population of 280,000 and is run by a forward-thinking Foundation Trust. This palliative medicine post is hosted at St Nicholas Hospice Care, on the West Suffolk Hospital site.St Nicholas Hospice Care is an independent charity providing palliative care to the patients and families of the West Suffolk and Thetford area. We are looking for a committed doctor to join our consultant-led medical team. You would work across the full range of our clinical settings, including a 12-bedded inpatient unit and community team. You would provide specialist palliative care, and support our approach to equip our community to be better prepared for the end of life.Situated in the heart of East Anglia, the cathedral town of Bury St Edmunds is a pleasant place to live and work.This advert may close earlier than advertised.Main duties of the jobWard block:o Daily ward round of in patients who need review that day, performing holistic assessments of palliative care and general medical needso Prescribing medications as appropriate, including specialist palliative medications, ‘end of life’ medicationso Admitting new patients, including holistic assessment and full clinical examinationo Arranging dischargeso Organisation and review of investigationso Liaising with other specialtieso Arranging transfer to hospital for acute treatment, if requiredo Liaising and communicating with patients’ families at all stageso Updating the electronic healthcare recordso Participation in handover meetingso Arranging after death paperwork (referrals to Medical Examiner, Coroner, and completion of Medical Certificate of Cause of Death)o Working with/supervised by palliative medicine consultants, Senior Hospice Physicians, and Advanced Nurse Practitioner. All these staff are experienced and will provide induction ‘in the moment’ teaching will be provided where appropriate.o Clinical supervision of junior trainees.Community Block:o Based in community team officeo Visits to patients at home, with other members of the MDT and alone where needed.o Managing a caseload of patients, jointly with community teamo Telephone advice to patients and other healthcare professionalso Liaising with GPs and other healthcare professionalso Seeking advice as required from senior doctor/consultant as requiredo Participation in weekly case review meetingsAbout us#BeKnown at West Suffolk NHS Foundation Trust. Job responsibilitiesPlease read the Job Description and Person Specification attached for full details of the postRelevant to both blocks: Working as part of the MDT with rest of medical team, on call consultant if relevant, nursing staff and allied healthcare professionals, including Psychological Services Team Participation in weekly hospice-wide MDT meeting Clinical supervisor will provide time for meetings at beginning, middle and end of placement, and monthly sessions between as needed Trainee to lead on their workplace assessment needs, but will be supported by hospice Time to get involved with clinical audit and other quality improvement projects will be supported and encouraged Weekly tutorial time with consultant offered, trainee-led, based on individual learning needs Participation in weekly medical team meeting, including presenting on relevant topics, and journal club Managing the rota for the weekly medical team meeting Supporting and teaching medical students SPA time will be allocated Opportunities for management experience will be encouraged, such as taking part in clinical governance meetings and taking a lead in certain areas1-in-6 ON CALL DUTIESWeek nights:From 5pm 9am, be available for any medical needs the ward might have, as during the day. Providing you live within a reasonable distance, you may return home and be contactable from there. (If your driving time is over 45 minutes, please discuss with the consultant.) Duties include: Admitting any patients who arrive during that period (most admissions are planned for daytime but emergencies or late arrivals due to transport delays occasionally need clerking during the out of hours period) Giving phone advice to nursing staff Returning to the hospice to see patients who have deteriorated unexpectedly and need a review of their care plan Some medications can be prescribed remotely if the patient does not need review in person The nursing staff or on call manager may ask you to attend for other reasons, if they deem they need support in a matter It is wise to phone the nurses around 10pm or before you go to bed, so that any small niggles or questions can be resolved this may avoid the nursing staff having to ring and wake you in the night There is a consultant on call available at all times in the out of hours period, on the phoneWeekend days: Attend the ward and do a ward round for any patients who need to be seen. These will either have been identified on the Friday by the ward medical team (or yourself), or be identified by either the nurses, or the patient/family may request to see you. Not all patients may need to be seen, e.g. if purely awaiting discharge Completion of the Medical Examiner Referral and Medical Certificate of Cause of Death for patients who have died over the weekend period. Please bear in mind the potential need to do this when conducting a ward round ensure you have at least met all patients for whom certification may be necessary. (Patients who have died over the weekend will have their death verified by the nursing staff.) Once all anticipated duties are completed, the rest of the on call may be done from home, returning that day only if neededPerson Specification
