We warmly welcome your application to join our friendly, award-winning Fylde Coast palliative and end of life care team and a well-established Fylde Coast End of Life Group with a longstanding collaborative approach to the care and support for our community.We are looking to recruit a full-time consultant to be based across our hospital in-patient and community services to complement the roles of our other consultants and medical teams and skilled palliative care team, to ensure continuity of care across settings and enable us to sustain and continue to develop our services in line with our Fylde Coast Palliative and End of Life Care strategic goals.The post holder will have an NHS consultant contract with Blackpool Teaching Hospitals NHS Foundation Trust. An additional honorary contract will be held with Trinity Hospice and the post holder will report to the Medical Director of Trinity Hospice.Main duties of the jobThe post holder willWork collaboratively with the four other Consultants in Palliative Medicine and the wider multi-professional team to ensure the delivery and continuity of specialist palliative care for the population of the Fylde CoastParticipate in the second on call medical rota to support the integrated adult service and 24-hour specialist palliative care advice line.About usAs the palliative care hub for the Fylde Coast, Trinity Hospice has been providing excellent specialist palliative and end of life care for adults and children with progressive life limiting illnesses for over 35 years. Job responsibilitiesWork collaboratively with the four other Consultants in Palliative Medicine and the wider multi-professional team to ensure the delivery and continuity of specialist palliative care for the population of the Fylde CoastTake an active role in the planning, development, and delivery of services to achieve service objectives and continuous improvement of wider specialist palliative care services.Provide direct clinical responsibility for the care of hospice inpatients, outpatient clinics and community visitsParticipate in weekly Specialist Palliative Care Multidisciplinary Team meetings.Support the delivery of best practice in palliative and end of life care in line with the Fylde Coast End of Life Care StrategyWork with the Fylde Coast Strategic End of Life Group to support the implementation of the local strategy and the ongoing development of palliative and end of life care across health and social care on the Fylde Coast.Provide strategic palliative and end of life care education leadership for the hospice and Fylde Coast along with key staff at the hospice and Blackpool Teaching Hospitals.Contribute to the delivery of education to all staff groups and act as a clinical or educational supervisor to speciality doctors and trainee Advanced Clinical PractitionersPerson Specification
