Salary: $93,070 – $116,337 per annum (To commensurate with experience and consistent with UHN compensation policy)

Working with our partners to bring life to great ideas, we use these ideas to create change through sustainable, shareable practices.:POSITION SUMMARY:The Project Manager position reports to the Senior Program Manager of Connected Care, Clinical Operations, and will be responsible for leading the implementation of a variety of activities such as vaccines, chronic disease management (COPD/CHF/Diabetes), and any new funded opportunities to advance the care for our patients and community members. We are looking for a talented and passionate individual to work alongside the Connected Care, Clinical Operations team as we expand and sustain clinical models of care.We work closely with the integrated care team aiming at improving the patient, caregiver and care provider experience, with a focus on a creating a seamless care experience for the patient across the health continuum. We collaborate with healthcare providers and system administrators from partner organizations to improve patient transitions from UHN into the community. Our goal is to improve patient satisfaction and their overall health outcomes by following the patient throughout their journey, including wherever possible providing needed care at home.This role will have a breadth of experience in delivering transformative care solutions to a variety of stakeholders, and is able to lead teams through all phases of the project lifecycle. The Project Manager should have strong change management and facilitation skills, navigate ambiguity with ease, and finds clarity for the team.RESPONSIBILTIES INCLUDE:
